Scoliosis: An Introduction
When the body is viewed from behind, a normal spine is straight without much disparity from side-to-side.Scoliosis is a condition that is often associated with a lateral, or side-to-side, curvature of the spine.This disorder frequently gives the appearance of the individual leaning to one side but it should not be confused with bad posture. Scoliosis is a troublesome deformity that is characterized by both lateral curvature and rotation of the vertebra oftentimes causing a symptomatic “rib hump” in the mid or thoracic spine. This is produced by the vertebrae in the area of the major curve rotating toward the concavity and pushing their attached ribs posterior thereby creating the characteristic rib hump seen in thoracic scoliosis. The pulmonary and cardiac functions can be interfered with if the thoracic curve and rib rotation exceeds 70 degrees. This intensity of curve and consequential cardiac and pulmonary changes are oftentimes seen later in life in untreated severe idiopathic infantile and juvenile scoliosis patients and, more often than not, present a threat to life.
Anatomy
If a person were to look at the trunk from a side view, the spine would reveal four normal curves: the cervical, thoracic, lumbar, and sacral. In the lower spine there is a normal “C-shaped” curve called swayback or lordosis, while the thoracic curve in the chest area has a “reverse C” called a kyphosis. Increased kyphosis in the thoracic area is called hyperkyphosis, while increased swayback is termed, hyperlordosis. Diversions from normal that are visible from a side view regularly accompany scoliosis changes. Postural exercises can eliminate some round back deformities that are simply due to poor posture. A small portion of people with kyphosis have more rigid deformities than the postural type, which are seen in conjunction with vertebral deformity. This class of deformity, called Scheuermann’s kyphosis, is much harder to treat than postural kyphosis, and it’s cause is unknown.
Almost anyone can help to identify a child or grownup with scoliosis just by observing the person in a standing position, preferably bare-chested and in boxers, and observing the following:
- One shoulder may be raised than the other.
- One scapula (shoulder blade) may be raised or more conspicuous than the other.
- There may be more space between the arm and the body on one side when the arms hang loosely at the side.
- One hip may appear to be higher or more conspicuous than the other.
- The head is not aligned with the pelvis.
- When the person is observed from the rear and asked to lean forward until the spine is horizontal, one side of the back seems higher than the other.

The most prevalent kind of scoliosis is, by far, Idiopathic, and although there are a variety of roots and many kinds, Idiopathic Scoliosis accounts for approximately 85% of all cases. “Idiopathic” means “no known cause” and is witnessed with equal occurrence in boys and girls in the mild or low curve magnitudes. Depending on the age of onset, this condition can be sub-classified into infantile, juvenile and adolescent categories. Idiopathic Scoliosis often runs in families and may be caused by genetic or hereditary influences. It is vital that if a scoliotic curve is discovered in a growing adolescent, the curves be monitored for any change by a periodic examination and on occasion standing x-rays. In ninety percent of instances, the scoliosis is mild and does not require active treatment, however increases in spinal deformity necessitate evaluation to determine if a brace or other treatment is necessary.
